Dr. Jordan Gardner – Medical Director of Telehealth
Dr. Jordan Gardner is a board-certified adult and child/adolescent psychiatrist. Dr. Gardner participated in the prestigious Medical Scholars Program, the 7-year combined B.S./M.D. program at the University of Miami Miller School of Medicine. He completed his adult psychiatry residency, as well as his child/adolescent psychiatry fellowship, at the University of Colorado School of Medicine.
Dr. Gardner offers comprehensive treatment for depression, anxiety, bipolar disorder, schizophrenia, post-traumatic stress disorder, personality disorders, autism spectrum disorder, attention deficit hyperactivity disorder, and child-hood behavioral disorders. He currently provides consultation and direct care services into integrated primary care practices via telehealth to outreach underrepresented communities in the state of Colorado.
Lauren Hahn – Clinical Manager of Telehealth
As a Licensed Professional Counselor (LPC), I am passionate about helping individuals navigate life's challenges and build meaningful, lasting change. My experience spans crisis intervention, juvenile corrections, substance use treatment, and outpatient therapy, providing me with a broad understanding of the unique struggles people face at different stages of life.
I work with children, adolescents, and adults, tailoring treatment to each person's individual needs, strengths, and goals. My approach is grounded in evidence-based practices, including CBT, ACT, SFBT, DBT and Motivational Interviewing. I strive to meet clients where they are in their journey, creating a safe, supportive, and nonjudgmental environment where they can explore challenges, develop coping skills, and work toward personal growth. I believe that meaningful change happens through collaboration, compassion, and a strong therapeutic relationship.
Evelyn DeLaCruz-Jiron – Telehealth Clinician
Dr. Evelyn DeLaCruz-Jiron is a Licensed Marriage and Family Therapist and Licensed Addictions Counselor with a Doctorate in Education and an emphasis in organizational psychology. As a Telebehavioral Health Clinician with ACS, she provides psychotherapy for individuals, families, and couples, specializing in trauma-informed care, family systems therapy, and solution-focused interventions. She is certified in EMDR, EFT, Experiential, CBT, ACT, and DBT interventions and is committed to helping clients strengthen communication, improve family cohesion, heal from trauma, and rebuild trust within the family system.
Jennifer Rodriguez – Telehealth Clinician
Taking the first step toward therapy can feel overwhelming, but you don't have to have it all figured out before you begin. My goal is to provide a warm, supportive space where you can explore life's challenges, gain insight, and move toward meaningful change at a pace that feels right for you.
With over 24 years of clinical experience, I have worked with children, adolescents, and adults in therapeutic preschools, schools, inpatient behavioral health, substance use treatment, and outpatient settings. My approach is compassionate, authentic, and collaborative, helping clients make sense of their experiences, build on their strengths, and navigate recovery and personal growth.
Outside of work, you'll find me hiking and camping with my husband, spending time with our son, relaxing with a good Victorian-era movie, and enjoying life with our two cats. I also have a collection of sloths that remind me of the importance of slowing down and taking life one step at a time.

Melanie Creach – Telehealth Clinician
Melanie Creach, LCSW, works with children, adolescents and adults. She earned her Bachelor of Science in Psychology from Regis University and her Master of Social Work from the University of Denver. Since 2015, Melanie has worked across hospital, care management, and outpatient therapy settings, bringing a holistic perspective to mental health care.
Melanie takes a collaborative, client-centered approach to therapy, drawing from evidence-based practices such as CBT, Motivational Interviewing, Narrative Therapy, SFBT, and ACT. She believes therapy should be tailored to each person's unique needs and is committed to using current research and best practices to support meaningful growth and change.
