Who Can Access Services?

AccessCare Services provides telehealth behavioral health support to eligible individuals who:

  • Are enrolled in Colorado Access Medicaid

  • Are enrolled in Colorado Access CHP+

  • Are enrolled in Denver Health Medicaid Choice

  • Reside within RAE Region 4 (Adams, Arapahoe, Denver, or Douglas Counties)

If you are unsure whether you qualify, our team can check whether you have active Colorado Access Medicaid or CHP+ coverage and connect you with the appropriate resources.

AccessCare Services Virtual Care Collaboration and Integration (VCCI) program provides assessment, therapy, counseling, psychiatric consultation, and family support through telehealth care. Services are available for children, adolescents, adults, and families.

Before Your First Appointment

To help ensure a smooth appointment experience:

Complete Required Forms

Patients may be asked to complete:

  • Telehealth_SMS Consent Form

  • Release of Information Form

  • Mandatory Disclosure Statement

  • Screening questionnaires such as PHQ-9, GAD-7, Vanderbilt Assessment

Prepare for Your Telehealth Appointment (How to Join a Zoom Meeting) word document

  1. Check your email for your appointment invitation.

  2. Click the Zoom meeting link.

  3. Join audio when prompted.

  4. Wait for your provider to begin the session.
